Who can refer to Cognivate Rehabilitation?
We accept referrals from case managers, solicitors and other legal professionals, healthcare professionals and other organisations involved in a person’s rehabilitation. We can provide individual disciplines or coordinate an interdisciplinary rehabilitation team according to the person’s needs.
What is community neurorehabilitation?
Community neurorehabilitation supports people with neurological injuries or conditions in their everyday environment rather than requiring rehabilitation to take place in a hospital or clinic. Following brain injury, this can involve working on cognitive, psychological, communication, physical and everyday functional difficulties, with rehabilitation focused on meaningful goals and participation in daily life.
How does Cognivate's interdisciplinary approach work?
Where someone requires input from several clinical disciplines, our neuropsychologists, occupational therapists, speech and language therapists and physiotherapists can work together as an interdisciplinary team. The team shares rehabilitation goals and coordinates intervention so that different aspects of the person’s recovery are considered together rather than in isolation.
